The Practice Change Fellows program seeks to develop quality leadership that can improve proper care for older adults. The fellowship is offered to nurses, physicians, and social workers over a two-year period so that participants can develop expertise to impact the quality of care for older adults.
Fellows complete projects designed to implement new geriatric programs and services that can be integrated within their existing organizations. They will attend three national meetings each year that will help them achieve the goals they establish for their fellowship period. In between these meetings, fellows will develop their new programs in conjunction with a national network of mentors and selected experts.
