Program Description

SAFE-TI is designed to provide senior living and long-term care communities knowledge and tools needed to implement a comprehensive approach to fall reduction. This approach includes a one day train-the-trainer educational session that provides a hands-on approach to using the program toolkit.

Participating communities will use the toolkit to develop comprehensive fall reduction programs in their communities resulting in decreased fall rates.

Unique features of the toolkit include:

• plans for developing exercise programs designed to improve gait and  

• balance
• instruments designed to assess fall risk and other factors that contribute

• to falls
• plans for interventions based on risk level
• tools for tracking QI data related to falls
